Service is sometimes included ('servizio'). If not, round up or leave 1–2€ per person. Never 20%.
Many trattorias add a 'coperto' (cover charge, 1–3€/person) — that's bread and table service, not a tip.
For truly exceptional service, 5–10% is generous. Italian wait staff are professionals on stable salaries.
